Get seen: This is most suspicious for epididymitis. Any new mass in the scrotum needs to be seen, both for diagnosis and treatment, and especially to rule out tumor which is especially concerning in a man your age.

Epididymitis?: An infection or inflammation of the epididymis can cause intermittent pain. A varicocele or enlarging of the testicular veins may cause intermittent pain (usually heavy sensation). Also intermittent torsion (twisting) of the testicle on its blood supply can cause this type of pain. Please see your doctor a simple exam can usually determine the cause.
