How can i have a ultrasound in the lower extremity that revealed venous insufficiency in the saphenous vein but no varicose vein?

It is early: Early in the development of varicose veins, before visible varicose veins actually begin to appear, we will sometimes see saphenous vein insufficiency (reflux) when we look with ultrasound. Otherwise, without ultrasound, you might not have any idea that anything was going on. With time, gravity and the reflux in the saphenous vein will cause visible varicose branches to develop.

Veins are deep: The saphenous veins in the legs may lie very close to the skin, or a little deeper, covered by fat. If deeper saphenous veins have insufficiency, the varicosities may not be visible because they are the deeper kind.

Saphenous vein Weak: A vein ultrasound can reveal early vein dysfunction in the saphenous vein (venous insufficiency with valve dysfunction) before it becomes clinically evident. The most common visible physical finding with saphenous reflux is either varicose veins or leg swelling. If the study is done early on in the condition before the dysfunction is severe, you can see the insufficiency without varicose veins.
