BPH with BOO: Most likely benign prostatic hypertrophy with bladder outlet obstruction. Basically a large prostate gland that is pinching off the urine tube. There could be other causes, but common things are common. He should see someone to make sure it is not cancer and consider getting on a medication or having surgery to help this.
