Change your life: Sorry, but there is no plan or chart. The only path to sustained weight loss is a commitment to change your life. Become an active person, engaging in vigorous activity for 4-5 hours per week. Stop eating all junk food, fast food, sweetened beverages. Emphasize fruits and vegetables in your diet. Don't think of this as a "diet"--it's your life, permanently. It works, and you'll feel better.
