Why?: If the toe is rubbing on your shoe or hurts, surgery is a reasonable alternative. If you are only concerned about appearance, think carefully before undergoing surgery. There is always the chance of infection, scarring or malalignment which could leave you with a result which is less appealing than the toe before surgery. Any surgeon who promises a perfect result probably has his fingers crossed.

Yes : 80% of people have a second toe longer than the first toe. Most shoes are made to accommodate this fact. Occasional the second toe is so long or the shoe wear ill fit that it causes the second toe to become deformed. There is surgery to correct this. Shorten the second toe for cosmetic reasons alone is not advised.

Yes you can: It is called a toe shortening procedure. There are times when if no pain is present docs don't like to operate. But, for a procedure that is commonly performed when there is pain, i think more docs are amenable to doing the surgery.
