Local reaction: Could be a local inflammatory reaction to something in your diet or anything else you may have consumed. I would first start by evaluating if you have changed any dental products, sometimes ingredients in these can cause a reaction. If it is not gone in roughly 14-21 days, i would have your dentist evaluate the area.

Decalcification: The white spot you descibed sounds like the initial start of tooth decay. If dental plaque is not removed particulary at the gume line the acid attack from the dental plaque begins the breakdown of the tooth enamel, as the enamel begins to breakdown this can be sensitive or painful. If the enamel has not been totally penetrated there are some new and exciting alternatives to reverse this process.

Over rushing: It seems to me that you are brushing the molar area too vigorously and starting to carve into the root part of the tooth that does not have enamel to protect it. The white spot may be lichen planus, but without seeing it, it's hard to offer a definite diagnosis.
