Mostly by history.: There are no reliable biological tests for diagnosing any psychiatric disorder, nor do psychological tests suffice. Clinicians use a combination of personal and family history plus current signs and symptoms to determine if someone has such a diagnosis. The "rules" are in the diagnostic and statistical manual, which is about to change to the 5th edition in 5/13. Doctors aren't always sure.
