Is it fungus?: I don't treat fingernails, but I do treat toenail fungus. If the disorder doesn't respond to an anti-fungal, it's likely not fungal. A nail culture should be performed to confirm the presence of fungus before any more medication is prescribed. Several non-fungal conditions, like psoriasis, can mimic fungal nails.

Effective tx: Possible effective treatment. Continue topical medication until finger nail grows out without fungus.

Sample to lab.: Nail fungus can be very difficult to cure 100%. You may want a doctor to test the nail by taking a sample and sending it to a lab to see if it is a callused or traumatized nail or if it is indeed fungus, if 4 years of antifungals did not work to improve the nails.
