Many things: A punch biopsy is a simple painless procedure that a podiatrist or dermatologist could do. It would take the guessing part out of the equation.

Patches: You should see a podiatrist or dermatologist; they might be able to determine if a biopsy is needed. The condition might resolve with topical medication alone.
