Many causes: Multiple conditions: early diabetic kidney disease, HIV associated nephropathy, hydronephrosis, cystic kidney disease, amyloidosis, granulomatuous kidney disease (sarcoidosis, tb).

Many things: 1 kidney: infection, compensatory hypertrophy related to a small other kidney, obstruction of the drainage system 2 kidneys: diabetes with early kidney enlargement, myeloma and amyloid involvement in the kidney, bilateral infection, acute injury to the kidney with swelling, obstruction of the bladder ask your doc for a visit to go over the films.
