Either way: If you are medically stable for general anesthesia, you can request it for skin cancer excision. The vast majority of patients are awake for this type of surgery, but occasionally patients who are very anxious will request to be put to sleep for this surgery.

Either: It depends on patient's request and doctor's discretion. If a lesion is small, it usually done under local anesthesia alone. If it is too large or needs multiple excisions followed by reconstruction, ti may be done under sedation or general anesthesia.
