Hard to say: Could be bad reflux with pain from the acid being improved by having food to "occupy" the acid. It can also cause esophageal pain / spasm. Try Prilosec (generic ok) in evenings for 10-14 days and see if resolves situation. If not, see dr for exam & to consider other causes.
