Could be: Laryngopharyngeal reflux manifests as a sensation of a ball in your throat, a need to clear your throat and can be associated with gastroesophageal reflux. It can also produce hoarseness. I can't comment on the fatigue and chronic pain. Have an ENT perform a fiberoptic nasopharyngoscopy to confirm the diagnosis.
