How is done heart ergometry at persons which are unable undergo this test on exercise bike and/or treadmill due to handicap their lower limbs?

2 main options: Either exercise using the arms on a bike-pedal device, or more commonly, medications are used that simulate exercise effects on the heart, & heart response / ECG monitored, sometimes including echocardiogram. (chemical stress test or echo).
