H. pylori: Definitely get tested for h. Pylori. In folks who are infected with this bacterium, treating it with an antibiotic cocktail can sometimes provide significant improvement in their symptoms. In the mean time, you should continue taking the Nexium (esomeprazole) as it was controlling your symptoms effectively.

Surgery is an option: Patients with gerd should be formally seen by a gastroenterologist. If a patient has relief with a ppi such as Nexium (esomeprazole) that can be continued. If despite remaining on therapy with a ppi the symptoms persist an upper endoscopy is indicated and consideration for referal to a surgeon to have an antireflux proceedure should be offered. Surgery, can often obviate the need for life long ppi treatment.
