Possibly thyroiditis: The information is incomplete, but subacute thyroiditis (sat), an inflammation of the thyroid gland can cause hyperthyroidism and high esr. People with sat have severe neck pain. With no pain sat is unlikely. A high ESR is found in many non-thyroid conditions: one can have mild hyperthyroidism and another cause for a high esr. Some people with non-thyroid illness have low TSH without being hyper.

Need to talk to doc: For something as specific as what you are describing, you need to talk to her doctor about diagnosis. A physician looks at the whole patient to make a diagnosis, not just a set of lab values.
