See Surgeon: Pilonidal cysts often are recurrent problems, and are often best managed by excision. This is usually done by a surgeon as outpatient/office procedure. If yours won't drain, at a minimum, it needs a dr to open it, otherwise the abx won't fully resolve the problem.

Surgery: This needs to be drained by a surgeon to resolve the infection. It can be excised and closed at another time once the infection is resolved.
