Many thoughts: Risky for older pts with alzheimer's to get sick as many eventually succumb to pneumonia, and the shaking could be rigors as a response to her underlying infection. She may not be capable of mounting much of a fever. Might also wonder about periodic seizure activity, as the infection may promote this. At this point, contact the family doctor and get advice.
