It may never: Large rotator cuffs, when left alone, do not heal on their own. Some of the inflammation may go away and the should can adapt to the changes, feeling better over time. After a rotator cuff repair, large tears have between a 30% and 70% chance of healing (the studies vary a lot). If it does heal, it takes around 3 months. Otherwise, your body may adapt to the shoulder a feel better.

Don't heal on own: The rotator cuff is like a rubber band under tension, any tearing results in pulling apart of the fibers and shortening of the muscle so even if scar tissue fills the gap the muscle hasn't been pulled back out to length to restore function and strength.
