Maybe: Sounds like your mother has a pretty significant fracture involving the joint in the big toe. This toe is very important and the fact that she has been referred to an orthopedic surgeon suggests that her doctor feels the injury is significant enough to at least warrant evaluation by a specialist. The ortho doc will evaluate your mother's toe and look at the xrays. Surgery is a possibility.
