Depends: Whether it is necessary depends on hte clinical scenario. Coronary angiography is the gold standard to assess for coronary artery disease. It does have some risks but is relatively safe when done by experienced operators. There are other ways to assess for CAD (ct angiography, stress tests) but they are not foolproof.

No, yes, yes: Cath not usually necessary. Echocardiograms are usually used and show the heart well cath is invasive but usually safe.
