Not really: 4-5mm pockets is not good but at the same time it is treatable and can be made healthy with scaling and root planning. Sometimes p;acement of antibiotics into the gums also helps. But periodontal disease is stubborn so you may need multiple treatments to get this under control. Also make sure you have excellent hygiene at home, otherwise the time spent treating will be a waste.

Sorry I disagree: Antibiotics in the pocket are an invitation to the development of resistant bacteria. Scaling and root planing leaves the bacteria in the pocket and they just need a chance to regrow. Brushing and flossing are not enought for most people. I would try something that kills the bacteria, shrinks the pockets and stops the bleeding like the perio protect method (j clin dent 2012).

Early Intervention: Scaling and root planing is initial therapy aimed at removing infectious deposits (tarter) and bacteria from the roots of your teeth. Is it bad that you need it? It means you are losing attachment (bone) around your teeth. The procedure itself involves numbing the gums and basically doing a very deep, thorough cleaning (same instruments as in a regular cleaning though).
