Probably: Bleeding from the mucous membrane of the septum between the nostrils. You may benefit by taking a cue tip and gently painting a little vaseline on the scab, particularly if it is in the front of the septum.

Need MD evaluation: Depends how long you have it. If it has been going on a while and the scab never heals, then need ENT specialist to see.
