A member asked:

Should a 3.2 cm thyroid nodule with an inconclusive biopsy be removed simply because of size?

Maybe: Most "inconclusive" thyroid biopsies are fine needle biopsies (fnas). "inconclusive" ones are usually benign but occasionally are not, and this can only be established with a proper surgical excision and full pathology examination. This is the reason to remove a nodule...Not its size.

Thyroid nodule: Yes! a nodule that large cannot be effectively biopsied by fine needle. Too many false negatives ie cancer is easily missed. Lobectomy or total thyroidectomy is the procedure of choice.

Depends: If the reason for the inconclusive biopsy is an inadequate specimen, and this is the first attempt, another needle biopsy should be performed. If this is the second attempt, or if the pathologist suggests there were adequate cells to analyze but no diagnosis could be made, surgery to remove the nodule is recommended. Intraoperative analysis, i.e. Frozen section, can be considered.
