What might make one w/connective tissue disease experience flares of brain feeling swollen, spacy &lightheaded when brain mri was normal?

MRI just one test: Mri would show severe inflammation or other problems if vasculitis or a stroke due to ctd were present. However, neuropsychiatric manifestations of ctds occur with normal mris. The mechanism is not well understood.
