Heart disease: Good question. One correction, heart disease, while still a major cause of death is on the decline. As we are able to protect people from dying from heart disease, they live longer, increasing chances ago dying from other causes such as cancer. Now, why doesn't it decrease faster? Because people still smoke, are overweight, don't control diabetes/cholesterol/high blood pressure...

It is decreasing but: It is decreasing , but slowly. The obesity and diabetes epidemics may stop or reverse this trend.
