High creatinine: Your results show you to have far advanced chronic kidney disease and possibly kidney failure. You should ask the doctor who ordered to the blood work to send you to a nephrologist who will order a 24 hour urine for creatinine clearance and protein and have blood taken when you return the urine. A renal ultrasound will also need to be done to see the size and morphology of your kidneys. Good luck.
