Pyelonephritis?: Back pain is an unusual symptom for a 3 yo to complain about. With fever, i would be concerned about a possible kidney infection. Other than these two symptoms, is he having other symptoms? Rash, diarrhea, cough or runny nose? I would call his doctor.
