Malabsorption: This is malabsorption / maldigestion until proved otherwise. If there's joint problems too, i'm thinking enteropathy arthropathy. The ANA may be a red herring. Solving this type of problem is why we have primary care physicians; respectfully nobody here can address this beyond raising possibilities. Celiac sprue? Whipple's? Crohn's? Many possibilities.
