Neurologic Problem: It sounds as if you troubles coming from your cervical spine. The nerve roots coming from the neck supply the arms and hands. Additionally, damage to the nervous system can either cause pain, pins and needles (paresthesias), numbness, and/or weakness. TOS: Thoracic outlet syndrome (tos) causes similar symptoms and aching discomfort in same side shoulder. Numbness and tingling in hand is usually into 4th and 5th fingers. Neck pain is common. Best doctor to sort this out might be an osteopath. Many persons with tos wake one or more times each night with their entire arm numb and tingly. See your primary doctor to rule out angina pectoris.
