Caffeine: It has some temporary antagonistic effect on the sedation of antipsychotics. The agitation some of my patients get with lots of caffeine make it ill-advised. Smoking tobacco definitely lowers antipsychotic blood levels, however. That has been thoroughly researched. It can drop the medication levels by 50%.

Some antipsychotics: It depends on the antipsychotic, and what liver pathways are needed to process it. Caffeine can be both an inhibitor and an inducer of liver cytochrome 1a2 -- an enzyme which metabolizes the antipsychotic clozapine. In some cases, large doses of caffeine have competed with Clozapine for the needed enzyme -- resulting in greater Clozapine side effects. Zyprexa (olanzapine) could be affected similarly.

Depends: The core effect of all antipsychotics is to decrease Dopamine levels, but all antipsychotics have effects on other neurotransmitters as well. Caffeine is a stimulant. Though the exact mechanism of caffeine is not known, it is clear that indirectly it lincreases stimulatory neurotransmitter such as norepinephrine and likely Dopamine as well which would decrease effects of antipsychotics.
