Dental scheduling: This is a question best asked to your dentist. Some can, most don't since the hygienist will usually do the cleaning and the dentist will do the cavity filling. Check with your dentist and call around if you need it done that way.

Clean and fill : It is fine to have a cleaning and filling done at the same appointment. I like to do the cleaning first, then do the filling. There is no reason not to do both.

Perhaps: It's hard for an office to plan out the day, if they don't even know what you yet need. Those offices that do it the same day, are usually the reason the other people are sitting in the waiting room for an hour.
