MD visit: You need to be checked out by your oncologist. Imaging should be done to see if the rcc has returned and metastasized or whether you have something else going on.

Not gout: The pain back to front needs to first be evaluated by your oncologist. If gout was causing your foot swelling, it would be extremely painful.
