Yes: If you feel you need a specialist, your physician could refer you to one they know. Additionally, communication between to physicians (ie your medical history, etc) would be very helpful.

Pain management: You should discuss with your pcp regarding your pain. Pcps are great in treating painful conditions. If your doctor thinks you need more advanced treatment from a specialist, he/she can refer you.

They will perform a physical exam, learn about your symptoms, and possibly order some tests (emg, x-ray, mri...). Based on these, they should come up with a comprehensive plan to treat your pain.
