Perio surgery: It may be that your periodontal pockets are not maintainable despite your best efforts, or that you may have bone defects from previous periodontal disease. In such a case, you should consult a periodontist for pocket reduction surgery so that you have healthy and maintainable gums. Another possible cause of bleeding gums could be systemic disease, but this has a low likelihood.

Bleeding gums: First, make sure your teeth are cleaned on a regular maintenance schedule as determined by your dentist. Second, ask the hygienist to watch you floss. Many people think that flossing hard is better, but you may be cutting rather than cleaning your gums. If your teeth have been cleaned recently and the hygienist feels you are flossing correctly, you may need to be checked by a periodontist.

Periodontist: Firstly, your tooth is not bleeding, your gums are. It is a sign of gum inflammation or gingivitis. If pockets are deep, you may not be able to clean effectively yourself, and i suggest a consult with a periodontist whose specialty is just this sort of problem. Chlorahexadine is an anti-bacterial rinse and is not for chronic use... It has its down side: staining and loss of taste sensation are two.

Periodontal consult: Great advise from everyone else. Remember that bleeding is an indication of inflammation and unhealthy. You may have other factors going on like medications, mouth breathing, dry mouth or others. I would recommend seeing a periodontist, the specialist in periodontal, gum diseases.
