Size matters: The most important criteria in deciding what to do with a thyroid nodule is the size and the age of the patient. Nodules less than 1 cm are practically almost benign and usually are watched to check for growth. If they are bigger than 1 or 1.5 cm, most docs will get a fine needle biopsy under ultrasound guidance to check if the cells are normal or abnormal.

Needs FNA: This could mean malignancy, but not always. You should have a fine needle aspiration - a minor procedure done under local anesthesia. This will usually sort things out and determine if surgery is required.
