Now: Typically appendicitis starts with mid abdominal pain which is vague and crampy and then begins to migrate to the right lower quadrant and become more localized and defined, many times associated with nausea and fever. Hope this helps.

You should be seen: For abdominal pain alone you do not have to go to an er if you can be seen by your doctor or an urgent care in a reasonable amount of time however, if the pain increases to the point where it is painful to walk, cough, sneeze, ride over bumps in your car then you may have peritonitis. This would be best worked up in a hospital ed setting because you may need surgery.
