Need very strong Rx: Cystic acne that has not responded to the anti-androgen / estrogen medication that you have been taken will require medications that only a dermatologist can prescribe, and that will be unpleasant. I wish you luck. With acne this severe, your primary care physician may also want to evaluate you for some cause of hormonal disturbance / metabolic syndrome; aerobic fitness may be helpful as well.

Hormonal: Those deep tender cysts on the jaw and chin in adult women are always hormonal. First, i recommend bloodwork to ensure you do not have any tumors producing hormones (rare). Avoid sugars, add probiotics, fish oil (2, 000 mg daily), and only eat organic foods especially meats as the hormones in our diets can contribute. If this fails after 3 months, i will add Isotretinoin or spironolactone - rx.
