Nerve irritation : In all likelihood, either you are experiencing irritation of the nerves that run from your neck and shoulder to your hand. On the other hand (no pun intended), your diabetes, or some other issue could be affecting your nerves and causing a peripheral neuropathy that could be causing it. Either way, you should let your doctor know so the appropriate things can be checked.

Carpal tunnel?: It could be carpal tunnel syndrome. Diabetics are predisposed to it. Further history and an exam by your physician should be able to help diagnose it. Other less likely possibilities are cubital tunnel syndrome and diabetic peripheral neuropathy (though you didn't mention your feet tingling or being numb).
