Possibilities..: "stabbing" pain sounds pretty severe. So, my threshold for asking you to see an md is low. That said, structures around/near the area you describe are: muscles (small, easy-to-injure ones), bone, small ligaments, lung (pain "referred" to the surface from deeper structures), blood vessels (unlikely pain source), and nerves (originating from the neck area and innervating muscles you describe).

Seek medical attn: Stabbing pain in your back could just be musculoskeletal however it might be due to inflammation surrounding your lungs, heart or other organs (stomach, pancreas, etc) . A physician needs to evaluate you and decide if you need imaging or other studies.
