Unlikely: Risk of cancer in a thyroid nodule is about 5%. You can't tell by just looking at it on an ultrasound. The only way to investigate this short of surgery is to undergo an ultrasound guided biopsy. At the very least, you will need to check another ultrasound in a year to monitor for size changes.

Needs FNA: Most thyroid nodules in 40 year olds (especially women) are benign. But - we usually recommend a fine needle aspiration of any nodule greater than 1 cm or smaller if it has worrisome us findings. Fna is a very easy procedure - local anesthesia only, no sedation, so you can drive yourself and go back to your usual activities immediately. The boston area has several world renowed thyroid docs.

Depends: We recommend biopsying any thyroid nodule over 1cm to make sure not cancerous. So i would not worry about your 5mm nodule but just have an ultrasound guided fna (fine needle aspiration) of your 1.4mm nodule.
