See below: Did he have a submersion event (under water for a time and when emerged, coughing up water, etc.)? If so, go to the er now. The child will need to be watched for 6-8 hours for signs of an aspiration which may require treatment. If no such event, watch your child and if any respiratory issues evolve, go to the er.

See your doctor : The symptom of a chest cold/cough with phlegm could be a sign of a simple viral illness or a sign of something more serious. A high fever and lethargy, poor eating/drinking, would be concerning signs. I'd suggest taking your son to see your pediatrician or family physician to rule out potentially serious conditions such as pneumonia.
