Check progress/meds: During hospitalization, many medications are started or doses adjusted to get you through the acute illness and manage your chronic illnesses for the first few days at home after hospitalization. These medicines and doses may be more optimal for you now but they may not. Your doctor needs to check on the progress of your illness as well as the effects of your medicines.

For the follow up: It's usually necessary for you and your reffering doctor for y to be insure that your recovery is going well and without complications, and you complaint with the medication that has been prescribed to you to be completed at home.
