ACE inhibitor: I prefer starting with ace inhibitors as first line agents. They have few side effects and have multiple beneficial effects on the heart and blood vessels. Sometimes people require combination of these drugs with diuretics. If that doesn't work, diagnostic testing may be required to look for reversible causes of hypertension.

Consult MD: You have stage2 htn. Goal: below 140/90mm of hg. If dm or renal disease, less than 130/80 mm of hg. Need lifestyle modification (each down 5 mm of hg) aerobic exercise /daily, eat mediterranean diet, na restriction, limit alcohol, keep ideal bw. You will need drugs-start with arb or acei plus thiazide!
