Can spine x-ray diagnose small fractures? or do you need to get ct or mri anyway and skip x-ray? .

They can: Find small fractures but ct is much more sensitive and accurate. Interestingly fractures can be very difficult to see on MRI but the MRI can offer a lot of other types of information.

Sometimes: Small fractures may be difficult to see on plain radiographs particularly in the posterior parts of vertebrae, but good screening test for major problems ct exam is reasonable to visualize spine in multiple planes, transaxial, coronal , and sagittal. Resolution is greater. Somtimes subtle fractures can only be detected by bone scintigraphy especially slight compressions of vertebral bodies.
