Open is Best: Open communication in a calm, straightforward and non-judgemental way is best. If your job is the only source of income for the family then this is very important. You know him best; if you are unable to communicate in this manner with him seek professional help with a counselor to find better ways to communicate.

caregiving: Recommend this be approached in a non-judgmental manner. It may be a good opportunity for problem solving. Are there some things your spouse depends on you for that he can do for himself? Do you need the help of others to provide him care? Can loved ones or relatives help or is it time to augment with a professional caregiver who is salaried. Is respite care available.?
