No: Thickening of the heart muscle or hypertrophy may be caused by hypertension but other causes such as a genetic disorder with congenital hypertrophic myopathy need to be excluded. The echocardiogram is the usual way to determine if your heart muscle is thickened and the degree of regression of the thickening may be used to show the effectiveness of antihypertensive therapy.

No: Anything that makes the heart have to work harder can make the muscle bigger. Think of teh effect of lifting weights on your skeletal muscles. High blood pressure is one cause but a blockage in the aortic valve for instance could do the same thing.
