No-it will rupture.: The natural course of appendicitis is that the appendix will rupture if untreated, usually within 2-3 days of the process starting. This will typically lead to inflammation which spreads throughout the abdominal cavity (generalized peritonitis). On occasion, the body will wall-off this perforation and create an abscess.

No: Untreated appendicitis will worsen possibly rupture. Treatment can include antibiotics but ideally surgical removal. In the 21st century surgical removal is the gold standard but antibiotics can be use as an interim treatment.

Complications: Real appendicitis does not go away on its own. If left untreated, appendicitis will progress and perforate (release the infected/contaminated fluid) which is associated with more severe complicates such as abscess formation and peritonitis. It can sometimes be fatal.

No: Once appendix gets sick and inflamed it will get steadily worse and eventually rupture. Harry houdini died of a ruptured appendix.

Not worth the risk: While there are cases where the tip of the appendix (for example) may get infected and not become a full-blown appendicitis and someone will be found in retrospect to have survived without surgery, the risk of perforation, progressing to sepsis followed by multi-organ failure and death is too great. And there is no way to know early on which direction one's individual appendicitis will take.

Not usually: Once appendicitis starts, it usually progresses to an increasingly serious abdominal infection. Left untreated, one is likely to die. Current treatment consists of hydration, antibiotics, and appendectomy (either laparoscopic or open). Antibiotics alone are starting to be used for cases of early appendicitis with perhaps a 75% success rate. You can't ignore appendicitis. You have to treat it.
