H&P, tests: Through nuclear stress test, stress echocardiogram, echocardiogram, pet scan, ct scan, and ekgs. These tests are only used after a thorough history and physical that provides the indications for the appropriate test(s).

Heart diagnosis: To diagnose heart disease we use history, physical exam, ecg, chest xray, echocardiogram, various imaging and nuclear imaging studies and cardiac catheterization and coronary angiography. There are other tests also which i haven't mentioned.
