Yes. : Yes. This should be checked out. There are a number of structures in the scrotum. However if there is a mass on the testicle, it is especially important that you be evaluated.

Given : Given the locations, size and how long it's been there, this likely an epididymal cyst which is not serious but i cannot be sure without an exam. I urge you to visit with the urologist (outpatient not in the hospital) who can answer your question and reduce your worries. T.
